West Virginia Licensed Nurse Navigator

Job Summary

To support patient care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming patient referrals, assess needs, provide clinical dispositions, and facilitate referrals to primary and specialty care providers in a remote setting.

Key responsibilities

• Efficiently triage new patient referrals to ensure timely access to care
• Collaborate with a multidisciplinary care team to expedite patient workups and treatment plans
• Educate patients on clinical information, treatment options, and preventive care behaviors

Required qualifications

• Current unencumbered licensure as a registered professional nurse in West Virginia or through the enhanced Nurse Licensure Compact (eNLC)
• Three years of registered nursing experience
• Ability to obtain certification in Basic Life Support within 30 days of hire
• Experience in care coordination and outpatient/clinic settings is preferred
• Bachelor of Science in Nursing Degree (BSN) is preferred
