West Virginia Licensed Nurse Navigator

Job Summary

To support patient care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming patient referrals, assess needs, provide clinical dispositions, and facilitate referrals to primary and specialty care providers in a remote setting.

Key responsibilities

• Assess and facilitate coordination of multidisciplinary care while providing communication and education in a culturally competent manner
• Efficiently triage new patient referrals to ensure timely access to care and collaborate with the care team on patient management
• Document patient interactions and maintain comprehensive medical records, serving as a conduit between patients and healthcare providers

Required qualifications

• Current unencumbered licensure as a registered professional nurse in West Virginia or through the enhanced Nurse Licensure Compact (eNLC)
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support to be obtained within 30 days of hire
• Experience in care coordination and outpatient/clinic settings preferred
