West Virginia Licensed Nurse Navigator

Job Summary

Managing patient referrals and facilitating care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming referrals, educate patients on care options, and collaborate with multidisciplinary teams in a remote setting.

Key responsibilities

• Assess and facilitate coordination of multidisciplinary care across the care continuum
• Clinically triage new patient referrals to ensure timely access to care
• Document patient needs, goals, and preferences while providing communication between patients and providers

Required qualifications

• Current unencumbered licensure with the WV Board of Registered Professional Nurses or multi-state licensure through the eNLC
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support within 30 days of hire date
• Experience in care coordination and outpatient/clinic settings preferred
