West Virginia Licensed Nurse Navigator

Job Summary

To support patient care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming patient referrals, assess needs, provide clinical dispositions, and facilitate referrals to primary and specialty care providers, all while working remotely.

Key responsibilities

• Assess and coordinate multidisciplinary care, ensuring effective communication and advocacy for patients
• Efficiently triage new patient referrals to guarantee timely access to care
• Collaborate with care teams to expedite patient workups, treatment, and care transitions

Required qualifications

• Current unencumbered licensure with the WV Board of Registered Professional Nurses or multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support within 30 days of hire date
• Experience in care coordination and outpatient/clinic settings preferred
