West Virginia Licensed Nurse Navigator

Job Summary

Managing patient referrals and coordinating care, the full-time West Virginia Licensed Nurse Navigator will triage incoming referrals, educate patients on care options, and facilitate appointments, all while working remotely.

Key responsibilities

• Assess and coordinate multidisciplinary care across the continuum, providing communication and advocacy in a culturally competent manner
• Efficiently triage new patient referrals to ensure timely access to care and collaborate with the care team to expedite treatment processes
• Document patient needs and preferences, bridging gaps in care and addressing barriers to ensure effective treatment delivery

Required qualifications

• Current unencumbered licensure as a registered professional nurse with the WV Board or through the enhanced Nurse Licensure Compact (eNLC)
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support within 30 days of hire
• Experience in care coordination and outpatient/clinic settings preferred
