West Virginia Licensed Nurse Navigator

Job Summary

To support patient care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming patient referrals, assess needs, and facilitate connections to primary and specialty care providers while working remotely.

Key responsibilities

• Efficiently triage new patient referrals to ensure timely access to care
• Collaborate with multidisciplinary care teams to expedite patient treatment and manage care transitions
• Educate patients on diagnoses, treatments, and preventive care while documenting their needs and preferences

Required qualifications

• Current unencumbered licensure as a registered professional nurse in West Virginia or through the enhanced Nurse Licensure Compact (eNLC)
• Three years of registered nursing experience
• Bachelor of Science in Nursing Degree (BSN) preferred
• Certification in Basic Life Support within 30 days of hire date
• Experience in care coordination and outpatient/clinic settings preferred
