Job Summary

A company is looking for a Case Management RN.

Key Responsibilities

• Coordinate care and services for members with complex health care needs
• Develop and monitor member-centric care plans while educating members and families
• Document care management activities and analyze high-risk cases for effective care coordination

Required Qualifications

• Registered Nurse (RN) with current licensure in Wisconsin or current Compact License
• 4+ years of experience as a Registered Nurse in various clinical settings
• 2+ years of case management experience
• Strong knowledge of clinical care management processes and health plan operations
• Familiarity with insurance benefits and ability to manage a caseload independently
