Job Summary

A company is looking for a Health Coach to support members in their wellness journey through personalized coaching and behavior change facilitation.

Key Responsibilities

• Facilitate telephonic or virtual one-on-one coaching sessions using motivational interviewing and behavior change strategies
• Engage high-risk members to promote preventive services and build long-term relationships
• Coordinate referrals to medical, behavioral health, and community resources while developing individualized wellness plans

Required Qualifications

• Bachelor's or Associate's degree in health education, nutrition, exercise science, psychology, kinesiology, public health, or a related field
• 2+ years of experience in health coaching, wellness counseling, or comparable client support
• Experience in a health plan, managed care, or population health environment
• Familiarity with health tracking apps and telehealth platforms
• Bilingual proficiency strongly preferred
