Job Summary

A company is looking for a Licensed Mental Health Therapist (Telehealth).

Key Responsibilities

• Assess and diagnose behavioral health conditions, formulating treatment plans in collaboration with the health team
• Provide therapeutic interventions, psychoeducation, and support for referrals to specialty behavioral health care
• Document member care in EMR and participate in multi-disciplinary case conferences to ensure holistic care

Required Qualifications, Training, and Education

• Active LCSW, LMFT, or LPC license in Wisconsin
• 4+ years of outpatient experience with adults enrolled in Medicaid, Medicare, or dual-eligible
• 3+ years of experience in assessing, planning, and managing behavioral patient care
• 1+ years of experience practicing telemedicine
• Proficiency with electronic medical records and technology-based tools
