Job Summary

A company is looking for a Registered Nurse - SHP Care Management (Remote/Hybrid within member service area).

Key Responsibilities

• Assess, coordinate, plan, and implement case management services for individual medical episodes
• Evaluate the effectiveness of alternative care services to meet case management program objectives
• Conduct home visits and travel as part of the DSNP program and Model of Care

Required Qualifications, Training, and Education

• Graduate of an accredited school of nursing
• Five years of clinical experience in relevant healthcare settings
• Valid, unrestricted Wisconsin nursing license
• Must complete CPUR or CPUM certification and case manager certification within two years of hire
• Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire
