Wisconsin Licensed RN Care Manager

Job Summary

A company is looking for a RN Care Manager with a Condition/Disease Management focus.

Key Responsibilities

• Provide case management services including assessment, planning, facilitation, care coordination, and evaluation
• Review medical and behavioral test results, update care plans, and close cases when goals are achieved
• Educate members and families about case management, health conditions, and available resources

Required Qualifications

• Graduation from an accredited school of nursing
• Bachelor's degree in Nursing preferred
• RN licensure in the State of Wisconsin
• Four years of clinical health care experience as a RN required
• Case Management certification preferred
