Job Summary
Providing comprehensive case management services, the full-time Wisconsin Licensed RN Care Manager will assess, plan, coordinate, and advocate for members' health needs while working remotely with occasional travel to the corporate office in Menasha.
Key responsibilities
- Screen candidates for case management, completing assessments and care plans based on clinical guidelines
- Evaluate member needs and update care plans based on progress towards health goals
- Educate members and families on case management, health conditions, and available resources
Required qualifications
- Graduation from an accredited school of nursing
- RN licensure in the State of Wisconsin
- Four years of clinical healthcare experience as an RN
- Bachelor's degree in Nursing preferred
- Case Management certification preferred
