Job Summary

A company is looking for a Social Worker - Adult Inpatient.

Key Responsibilities

• Evaluate complex patient and family situations with legal, ethical, and psychosocial components
• Develop and monitor individualized care plans, addressing barriers to goal achievement
• Participate in discharge planning for patients requiring post-discharge services

Required Qualifications, Training, and Education

• Master's Degree in Social Work Required
• 2 years prior hospital-based social work experience Preferred
• 2 years prior hospital or community-based social work experience if supporting the HIV Clinic Preferred
• 2 years prior hospital or clinic-based pediatric experience if supporting American Family Children's Hospital (AFCH) Preferred
• Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) in Wisconsin within 90 Days Required
