Wisconsin Licensed Social Worker

Job Summary

To support kidney care patients, the part-time Wisconsin Licensed Social Worker will assist individuals and families in coping with long-term illness, conduct psychosocial assessments, and facilitate discharge planning, with two days onsite and one remote day each week.

Key responsibilities

• Assist patients and families in managing challenges related to severe or long-term illnesses
• Conduct psychosocial assessments and facilitate routine discharge planning
• Collaborate with multidisciplinary teams and direct patients to community resources as needed

Required qualifications

• Master of Social Work from an accredited school and current Wisconsin state licensure
• Advanced assessment and treatment skills, including individual, couple, and family therapy
• Ability to manage confrontational and stressful situations with professionalism and compassion
• Experience in developing educational materials and maintaining effective working relationships
• Sensitivity to racial and cultural diversity in patient care
