Job Summary
Supporting the ED Transitions of Care team, the full-time Wisconsin Licensed Transition Specialist will manage member transitions in the community, coordinating care plans and conducting assessments while working remotely.
Key responsibilities:
- Develop and oversee effective transition plans for members in collaboration with care coordination teams and providers
- Assist with member assessments and the transition process based on enrollment or care service needs
- Educate members and their families on services, requirements, and limitations related to transition planning
Required qualifications:
- Bachelor's degree in a related field
- 2 to 4 years of relevant experience in healthcare or behavioral health
- Familiarity with Wisconsin Medicaid
- Experience in care coordination or management
- Ability to manage a high volume of inbound and outbound calls
