Job Summary
Description:
On behalf of our client, a large managed health care plan, we are seeking an experienced registered nurse to provide supervision and coordination of day-day operations within the Disease Management Department. Reporting to the Manager of Disease Management, the Complex Care Coordinator is responsible for hiring, training and evaluating the performance of clinical associates and performs direct supervision of disease specialists to proactively identify the beneficiaries at risk and develop a plan of care for the assigned clinical condition. Some potential for intermittent telecommute.
Key Duties Include:
Implementing, coordinating, evaluating and modifying activities that support accomplishment of departmental goals.
Interviews, recommends for hire and evaluates staff performance.
Works collaboratively with other departments and communicates with providers, beneficiaries and community resources as needed to support the patient's plan of care.
Ensures the use of current and appropriate evidence-based guidelines in decision-making.
Performs day-day activities necessary to effectively evaluate the Disease Management program and makes appropriate modifications.
Qualification:
RN with minimum of two years disease management experience.
Master's Degree preferred or related education for disease specialty.
Previous supervisory experienced required.
4 years of clinical experience required.
Demonstrated knowledge of Disease Management Program goals and disease process management.
Strong leadership skills.
