Job Summary
On behalf of our client, we are seeking an experienced Registered Nurse to serve as Disease Management (DM) Specialist and provide education and resource health information services to health plan members. The Disease Management Specialist (RN) screens and determines the members needs and assists in identifying appropriate program goals and health education interventions.
The successful candidate utilizes available resources to assist with management of disease and works to empower the member to achieve better self-management of their disease.
The DM Specialist will work in a professional office environment as a team member of the Disease Management Department for a well-known managed health care plan.
The RN will reside in the Pacific Time Zone and telecommute from work Monday thru Friday during normal business hours. This is a full time position that is temporary for a period of two to four months with the intent to direct hire.
Duties and responsibilities include :
Demonstrates strong organizational and time management skills to meet departmental goals
Provides one-on-one and group telephonic disease management education
Preforms initial, on-going and post-intervention patient assessment
Collaborates with member(s) and providers to develop individualized care plan
Participates in Disease Management Quality Improvement efforts
Job Requirements:
Graduate of an approved program of Nursing; Bachelor's degree in Nursing preferred
Current licensure as a Registered Nurse
Strong interpersonal relationship skills
Minimum of 2 years' experience with disease specific education
Demonstrated knowledge of the disease process and its management
Ability to work independently and as a team member
Strong communication skills
Strong organizational and time management skills
Proficient in Microsoft Word, Excel, Access and IBM Lotus Notes

Required license or certification:
Active and Unrestricted Registered Nurse License in California and U.S. Citizenship
