A company is looking for a EAP WorkLife Analyst - Medicare Team.

Key Responsibilities:

• Provide education about community resources, conduct resource needs assessments, and make referrals
• Document member interactions and research, work in an inbound and outbound call center environment
• Complete outbound phone calls, research, and handle administrative and customer service tasks

Required Qualifications:

• 1+ year(s) of experience educating on community resources and completing resource needs assessments
• 1+ year(s) of experience working with elderly and/or adults with disabilities
• 1+ year(s) experience in social work or related field
• 1+ year(s) experience conducting resource needs assessments
• Bachelor's degree required in Social Work or related field, Master's degree preferred
