Wraparound Care Coordinator

Job Summary

A company is looking for a Wraparound Care Coordinator to provide intensive case management services to youth and families with mental health diagnoses or developmental disabilities.

Key Responsibilities

• Provide home-based and community-based case management services
• Develop individualized goals with youth and families to support their needs
• Attend staff meetings and participate in supervision sessions

Required Qualifications

• Bachelor's Degree in Social Work or related field
• Knowledge of serious mental illness, substance abuse, and trauma
• At least two years of experience working with families and children in the mental health system
• Ability to work flexible hours, including evenings
• Must have a good driving record and meet physical requirements for family visitation
