Wyoming Licensed Care Coordinator

Job Summary

A company is looking for a Care Coordinator, Remote.

Key Responsibilities:

• Serve as the link between patients and health care personnel to ensure smooth transitions of care
• Perform assessments and collaborate with health care teams to establish optimal discharge plans
• Engage with patients and families telephonically to facilitate informed decisions and monitor progress

Required Qualifications:

• Active, unrestricted registered clinical license in state of hire (RN, PT, OT, or ST)
• 5+ years of clinical experience, with at least 1 year in a Care Coordinator role
• May require multi-state licensure to meet business needs
• Experience working with the geriatric population preferred
• Background in patient education, rehabilitation, or home health nursing is advantageous
