Clearances as required by specific practice or hospital, as applicable. Proficiency in MS Excel and enhanced data and statistical analysis skills. Experience working with physicians in provider practice settings and members in their home.
Administering a needs assessment instrument to program participants. Recording data in SIS databases. Tracking multiple monthly assessment referrals. Position Requirements Include: Ability to travel as part of the SIS assessor team. 4-year college degree; work in the field of human services.
