Collects assessment-related data and entering needs assessment data. Comply with all applicable regulations and requirements (HIPAA, Medicaid, etc. ) Qualifications for this position include: Must attend monthly SIS team meetings as well as travel as part of the SIS assessor team.
Participate in quality improvement activities, including data collection, tracking, and analysis. Qualifications for this position include: Education MA/MS. Independent behavioral health licensure valid in State of California: (LCSW, LPCC, LMFT)
Administering a needs assessment instrument to program participants. Recording data in SIS databases. Tracking multiple monthly assessment referrals. Position Requirements Include: Ability to travel as part of the SIS assessor team. 4-year college degree; work in the field of human services.
