Job Summary

Supporting patients in a remote capacity, the full-time Arizona Licensed Care Coordinator will manage patient intake, treatment coordination, and care navigation within brain and mental health services while ensuring compassionate and timely care delivery.

Key Responsibilities

• Conduct patient screening calls and assist with intake processes, ensuring accurate documentation and risk management
• Coordinate treatment planning and appointment scheduling in collaboration with clinical teams and external partnerships
• Maintain compliance with clinical documentation standards and participate in team meetings for continuous professional development

Required Qualifications

• Compliance with all state board requirements, including fingerprinting and application submission
• Proficiency with telehealth platforms and remote clinical workflows
• Experience in a mental health clinic or psychiatric setting, with understanding of patient workflows
• Experience in a fast-paced call center environment requiring immediate follow-up
• Ability to work independently and collaboratively in an interdisciplinary team environment
