Arizona Licensed Care Manager

Job Summary

To support patient-centered care in the Ambulatory Care Management department, the full-time Arizona Licensed Care Manager will coordinate appointments, connect patients with essential resources, and manage health information exchanges while working in a hybrid remote environment.

Key responsibilities

• Coordinate patient appointments and perform proactive outreach for post-discharge follow-up
• Identify and overcome barriers to care while connecting patients with necessary referrals
• Escalate patient care needs to the clinic RN care coordinator and ensure timely outreach based on health plan updates

Required qualifications

• High school diploma or GED required
• Three years of experience in healthcare as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, or in data reconciliation
• Successful completion of Valleywise Health EPIC computer classes within three months of hire
• Valid fingerprint clearance card or completed Affidavit at the time of hire
• Current BLS certification from an approved American Heart Association training center within three months of hire
