Job Summary

To enhance patient outcomes, the part-time Arizona Licensed Clinical Pharmacist will manage medication therapy for patients in the Optum Insight ACO Practice Extend Program, collaborating with care coordinators and utilizing remote work flexibility.

Key responsibilities:

• Conduct Comprehensive and Targeted Medication Reviews for high-risk patients and those on expensive medications
• Engage with providers to deliver patient-specific drug-therapy recommendations and coordinate follow-up care
• Document all medication therapy management interventions accurately within the technology platform

Required qualifications:

• Doctor of Pharmacy (Pharm.D.) degree from an accredited School of Pharmacy
• Current Arizona Pharmacist licensure in good standing with no disciplinary actions
• 3+ years of direct patient care experience in Medication Therapy Management
• Bilingual in Spanish and English
• Ability to work flexible hours, including two evenings per week
