Job Summary

Seeking an Arizona Licensed High Risk RN, the full-time Remote Care Manager will coordinate care for Senior Health patients by developing action plans, managing transitions of care, and collaborating with care team members across multiple states.

Key responsibilities

• Participate in daily huddles and weekly rounds to review patient care needs and triage calls
• Create patient-centered action plans and manage care transitions, ensuring effective communication among healthcare teams
• Address post-discharge barriers and facilitate access to necessary resources for patients

Required qualifications

• AZ Licensed Registered Nurse (RN) with the ability to obtain licensure in additional states
• 5+ years of RN experience, including at least 1 year in care coordination or case management
• Experience in complex care settings, particularly with senior health and home-based care services
• Advanced knowledge of care management principles and chronic disease education
• Strong interpersonal communication skills and proficiency in using technology for patient care
