Arizona Licensed MTM Pharmacist

Job Summary

To support a dynamic clinical engagement team, the part-time Arizona Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document care delivered while working remotely on a contract basis.

Key responsibilities

• Engage members telephonically to schedule consultations and provide medication therapy management services
• Assess medication regimens for appropriateness and provide patient education on medication and disease states
• Document patient care notes and recommendations accurately while adhering to HIPAA regulations and organizational policies

Required qualifications

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely within a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Bachelor's Degree or Pharm D
