Arizona Licensed MTM Pharmacist

Job Summary

To support a dynamic clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document care delivered while working remotely with a flexible schedule.

Key responsibilities:

• Engage members telephonically to schedule consultations and provide medication therapy management
• Utilize prescription claims data to generate clinical recommendations and assess medication regimens
• Document patient care notes and participate in departmental meetings to enhance clinical programs

Required qualifications:

• Current and unrestricted Pharmacist license in the state of residence
• Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services
• 1+ years of experience working remotely in a cross-functional team environment
• Intermediate proficiency with Microsoft Word, Outlook, and Excel
• Bachelor's Degree or Pharm D
