Job Summary

Supporting members through the Heart Wise program, the part-time Arizona Licensed Registered Dietitian will provide nutrition education, establish SMART goals, and monitor progress via telephonic and video visits to improve heart health and blood pressure management.

Key responsibilities

• Conduct comprehensive nutrition assessments and develop individualized nutrition care plans for patients
• Provide nutrition education and counseling to patients, families, and healthcare team members
• Participate in quality improvement activities and collaborate with the interdisciplinary healthcare team to optimize nutrition outcomes

Required qualifications

• Bachelor's degree in Nutrition, Dietetics, or equivalent coursework
• Registered Dietitian or RD Eligible, with RD credentials required within 6 months of hire
• Current registration through the Commission on Dietetic Registration of The Academy of Nutrition and Dietetics
• Maintains current licensure as required by State law
• Ability to communicate effectively and manage a patient caseload independently
