Job Summary
To support the mental and behavioral health needs of members in Brevard County, the full-time Behavioral Health Navigator will coordinate care, conduct screenings, and provide education while working remotely.
Key responsibilities
- Coordinate care for members with therapy, psychiatry, and mental health resources, collaborating with external behavioral health services
- Conduct Social Determinants of Health screenings and provide resources and referrals to enhance member well-being
- Engage in community outreach to build partnerships with local organizations and support programs
Required qualifications
- Bachelor's Degree in Social Work, Psychology, Counseling, or a related field, or Certified Medical Assistant with relevant experience
- 1-3 years of experience in mental or behavioral health, care navigation, or community outreach
- Knowledge of area community resources and experience in conducting behavioral health screenings
- Experience in care coordination, referral management, or patient navigation
- Spanish speaking preferred but not required
