Job Summary
Providing essential support for member transitions in the community, the full-time Behavioral Health Transition Specialist will coordinate care plans, educate enrollees about services, and assist with member assessments while working remotely from Texas.
Key responsibilities
- Supports care coordination teams and providers in developing effective transition plans for members
- Assists with the transition process for members based on enrollment or changes in care services
- Educates members and their families on service requirements, limitations, and exclusions during the transition planning process
Required qualifications
- Bachelor's degree in a relevant field
- 2-4 years of related experience in behavioral health or care coordination
- Experience in patient or member-facing roles
- Strong understanding of care transition processes
- Ability to engage effectively with patients, members, and healthcare providers
