Bilingual Care Coordinator

Job Summary

Supporting proactive care coordination for patients with complex chronic conditions, the fully remote Bilingual Care Coordinator will manage patient outreach, maintain care plans, and ensure documentation compliance while working within Pacific Time hours.

Key responsibilities

• Conduct patient outreach and engagement through text and phone calls to enrolled patients, ensuring compliance with Medicare CCM billing requirements
• Support patient enrollment and onboarding activities, verifying eligibility and maintaining regular patient engagement
• Document patient interactions accurately within the CCM patient management platform and EHR systems, ensuring audit-ready documentation standards

Required qualifications

• Certified Medical Assistant
• Bilingual (Spanish/English)
• 1-3 years of healthcare experience, preferably in chronic disease or nephrology
• Strong clinical assessment and documentation skills
• Intermediate skills with MS Office Suite, including Outlook and Teams
