Bilingual Clinical Care LVN
Job Summary
To support a growing Remote Patient Monitoring Department, the full-time Bilingual Clinical Care LVN will conduct patient outreach through calls, monitor health alerts, and collaborate with healthcare providers while working remotely in a contact center environment.
Key Responsibilities
- Conduct outbound and inbound calls to check on patients and address health concerns, managing a call volume of 70 to 90 calls per day
- Monitor and respond to Remote Patient Monitoring alerts, escalating concerns as necessary
- Document all patient interactions in real time and perform monthly wellness assessments
Required Qualifications
- Active LVN/LPN license required
- Bilingual proficiency in English and Spanish
- Proficient with computers, EMRs, and telehealth tools
- Strong communication and organizational skills
- Preferred: At least 1 year of nursing experience, particularly in RPM, telehealth, or chronic care
