Bilingual Remote Care Coordinator

Job Summary

Working remotely in a full-time capacity, the Bilingual Remote Care Coordinator will assist members with care coordination needs, manage referrals and authorizations, and collaborate with the RN Case Manager to support seniors and those with chronic care needs.

Key responsibilities:

• Reaches out to members to assist with referrals, authorizations, and care coordination tasks
• Creates cases and completes assessments for hospital and skilled nursing facility discharges
• Establishes relationships with members, acting as a patient advocate and managing alerts related to their care

Required qualifications:

• Minimum 1 year of experience working in Health Care IPAs, including entering referrals and prior authorizations
• High School Diploma or GED; Bachelor's degree or four years of additional experience in lieu of education
• Bilingual in English and Spanish
• Knowledge of Managed Care Plans and Medi-Cal
• Basic computer skills, including proficiency in Microsoft Outlook, Excel, and Word
