Bilingual Remote LPN

Job Summary

Supporting patients enrolled in chronic care management and remote patient monitoring programs, the full-time Bilingual Remote LPN (Care Navigator) will provide education, conduct phone consultations, and maintain patient records while working remotely in a patient-centered environment.

Key Responsibilities:

• Complete phone consultations with patients to provide support and education about their chronic conditions
• Educate patients on health management strategies and assist them in achieving their provider-approved care goals
• Maintain accurate patient records and document all interactions and care plans per protocol

Required Qualifications:

• Current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care, cardiology, internal medicine, home care, or chronic care management
• Ability to communicate fluently in Spanish, both verbally and in writing
• Proficiency in using electronic health records (EHR) and care management software
• Reliable internet connection and proficiency with virtual communication tools
