Bilingual Social Care Coordinator
Job Summary
Supporting New York's Medicaid Section 1115 Waiver program, the remote Bilingual Social Care Coordinator will engage Medicaid members, assess eligibility for Health-Related Social Needs services, and coordinate referrals through Epicured and community partners.
Key Responsibilities
- Manage inbound and outbound member communications via phone and email using various technology platforms
- Conduct outreach to screen potential members and navigate them to service providers based on identified health-related social needs
- Track and follow up on referrals to ensure service delivery and member satisfaction
Required Qualifications
- High school diploma (Associate degree preferred)
- 3-5 years of experience in customer service, call centers, social services, care navigation, or related fields
- Experience in a virtual contact center environment (Five9 preferred)
- Strong communication and member engagement skills
- Ability to manage high-volume interactions in a compliance-driven setting
