Bilingual Spanish Care Coordinator

Job Summary

Supporting a transitions-of-care initiative, the full-time Bilingual Spanish Care Coordinator will conduct high-volume telephonic outreach to members following hospital and skilled nursing facility discharges, ensuring timely follow-up care and documentation in a remote setting.

Key responsibilities

• Conduct high-volume outbound telephonic outreach to members post-discharge, ensuring compliance with established outreach protocols
• Engage members to assist with scheduling follow-up appointments and coordinating care needs while documenting all outreach activities accurately
• Collaborate with clinical and care management teams to manage member needs and escalate any clinical concerns as necessary

Required qualifications

• Minimum 1 year of experience in healthcare, including telephonic outreach and patient interaction
• High School Diploma or GED, with completion of an accredited Medical Assistant program preferred
• Bilingual in English and Spanish
• Proficient in Microsoft Office applications and able to type at least 35 WPM
• Knowledge of managed care plans and Medi-Cal is preferred
