Job Summary
To enhance the ability of frail seniors to age in place, the part-time California Licensed Assessment RN will conduct telephonic assessments, develop care plans, and provide health education while working remotely during Pacific Time business hours.
Key responsibilities
- Perform initial and annual telephonic assessments for dually eligible members and determine Nursing Facility Level of Care
- Evaluate assessment findings to ensure clinical appropriateness of member-centered care plans and develop comprehensive care strategies
- Provide education and coaching for chronic conditions to promote member engagement and successful health outcomes
Required qualifications
- Associate's degree in nursing (ADN/ASN) required; Bachelor's degree in nursing (BSN) strongly preferred
- CA Registered Nurse (RN) license required
- 3+ years of experience in managed care, healthcare environment, or case management
- Bilingual in English/Spanish preferred
- 2+ years of experience working with seniors and remotely strongly preferred
