California Licensed Bilingual RN Case Manager

Job Summary

To support the care of members with complex and chronic needs, the full-time California Licensed Bilingual RN Case Manager will manage healthcare coordination and deliver nursing care remotely, ensuring access to appropriate services and facilitating communication among care teams.

Key responsibilities:

• Coordinates care and serves as a resource for members, families, and physicians
• Completes comprehensive assessments and implements care plans in collaboration with the care team
• Educates members on self-management and assists in resolving barriers to care

Required qualifications:

• Active, valid, and unrestricted RN license in California (Non-Compact)
• Minimum 2 years of clinical experience and 1 year of case management experience
• Completion of post-high school courses to obtain RN licensure or an Associate's Degree in Nursing
• High level of understanding of community resources and chronic condition management
• Proficiency in PC-based software programs, including Word, Excel, and PowerPoint
