California Licensed Bilingual RN Case Manager

Job Summary

To support a growing Medicare Special Needs Program, the full-time Remote California Licensed Bilingual RN Case Manager will manage healthcare coordination for members with complex and chronic care needs, ensuring access to appropriate care and facilitating communication among the care team.

Key responsibilities

• Coordinates care by serving as a resource for members, their families, and physicians
• Completes comprehensive assessments and collaborates with the care team to implement care plans
• Educates members on self-management and assists in resolving barriers to care

Required qualifications

• Active, valid, and unrestricted RN license in California (Non-Compact)
• Minimum of 2 years of clinical experience and 1 year of case management experience
• Completion of post-high school courses to obtain RN licensure or an Associate's Degree in Nursing
• High level of understanding of community resources and management of chronic conditions
• Proficiency in PC-based software programs including Word, Excel, and PowerPoint
