Job Summary
To support post-discharge members, the full-time California Licensed Care Manager will perform care management duties including assessments, medication reconciliation, and developing care plans, all while working remotely.
Key responsibilities
- Evaluates member needs through post-discharge assessments and medication reconciliation
- Develops care/service plans in collaboration with healthcare teams to support member transitions
- Facilitates education and resources for members and caregivers to enhance overall quality of care
Required qualifications
- Active California RN licensure is required
- Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
- 2-4 years of related experience in discharge planning or care coordination
- Experience in postpartum care is strongly preferred
- Licensure as LISW, LCSW, LMSW, LMFT, LMHC, or LPC is a plus
