Job Summary

To ensure optimal patient outcomes, the part-time California Licensed Care Manager RN will manage patient resources during admissions, communicate with medical staff and outside resources, and monitor medical records for compliance with clinical and financial standards.

Key responsibilities

• Manage patient resources throughout their admission process
• Communicate daily with medical staff and external resources to ensure appropriate resource utilization
• Monitor medical records for variances and intervene to achieve optimal outcomes

Required qualifications

• California Registered Nurse License upon hire
• 2 years of acute nursing experience or case management experience
• Environmental Orientation training due within 30 days of hire
• Case Management Competency training due within 90 days of hire
• General Nurse Competency training due within 30 days of hire
