California Licensed Care Manager RN
Job Summary
To support patient care, the full-time California Licensed Care Manager RN will manage case management functions for complex patients, coordinating care and resources remotely during day shifts.
Key responsibilities
- Plan, coordinate, and negotiate care for patients and their families throughout the continuum of care
- Communicate with medical staff and hospital personnel to ensure appropriate resource utilization according to clinical and financial standards
- Monitor medical records for variances and intervene to achieve optimal patient outcomes
Required qualifications
- Associate's degree in nursing or a nursing diploma upon hire
- California Registered Nurse License upon hire
- 2 years of acute care experience in a Medical Surgical or Inpatient setting
- 1 year of experience in care management or utilization review, or completion of TIPS or Case Management Orientation Program upon hire
