California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and determine eligibility for services
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams
• Monitor patient progress and address barriers to achieving treatment goals, ensuring adherence to regulatory guidelines

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing client-focused care plans and conducting patient assessments
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to collaborate with multidisciplinary teams and communicate effectively with patients and caregivers
• Familiarity with healthcare services and resource coordination for patient care
