California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop comprehensive case management plans
• Monitor patient progress towards treatment goals and ensure continuity of care through resource coordination
• Document patient cases accurately and maintain compliance with regulatory and accreditation standards

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in evaluating patient needs and developing care plans
• Knowledge of healthcare regulations and standards
• Ability to work collaboratively with healthcare teams and external agencies
• Proficiency in documenting patient cases and managing authorizations
