California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and determine eligibility for services
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams
• Monitor patient progress towards treatment goals and ensure continuity of care through effective resource coordination

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing patient-centered case management plans
• Knowledge of regulatory and accreditation guidelines relevant to case management
• Ability to assess medical necessity and required levels of care
• Experience in collaborating with healthcare teams and community resources
