California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, develop case management plans, and coordinate resources remotely while ensuring compliance with regulatory guidelines.

Key responsibilities:

• Evaluate patient needs and goals to develop client-focused case management plans in collaboration with healthcare teams
• Coordinate resources and services to ensure continuity and quality of care while monitoring patient progress toward treatment milestones
• Assist in identifying barriers to care and facilitate access to appropriate services and resources for patients and their caregivers

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in evaluating patient needs and developing care plans
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Proficiency in documenting patient cases and managing authorization data
• Ability to collaborate effectively with healthcare teams and external resources
