California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate resources, and develop client-focused care plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs, determine eligibility, and develop case management plans
• Monitor patient progress towards treatment goals and ensure continuity of care by collaborating with healthcare teams
• Document patient cases and assist in identifying barriers to achieving care goals, making appropriate referrals as needed

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of healthcare regulations and case management processes
• Experience in developing and implementing patient care plans
• Ability to collaborate effectively with multidisciplinary teams
• Proficiency in documentation and data entry related to patient care
