California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate needs, develop client-focused care plans, and coordinate resources remotely while ensuring adherence to regulatory standards.

Key responsibilities:

• Evaluate patient needs and develop comprehensive case management plans in collaboration with healthcare teams
• Coordinate resources and services to ensure continuity and quality of care for patients
• Document patient cases and assess progress towards treatment milestones and care plan goals

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating patient care and collaborating with healthcare professionals
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to assess medical necessity and determine levels of care
• Experience in documenting patient cases and managing authorization data
