California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and determine eligibility, benefits, and education for admissions
• Coordinate resources and services to ensure continuity and quality of care while monitoring patient progress
• Develop and communicate comprehensive disposition plans in collaboration with healthcare teams and patients

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing case management plans and coordinating patient care
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to document patient cases accurately in medical files
• Familiarity with healthcare resources and services for patients and caregivers
