California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused case management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and determine eligibility, benefits, and education for admissions while entering authorization data
• Coordinate resources and services to ensure continuity and quality of care, monitoring patient progress toward treatment goals
• Develop and communicate comprehensive disposition plans in collaboration with patients, caregivers, and healthcare providers

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of healthcare regulations and accreditation standards
• Experience in developing and implementing patient care plans
• Ability to collaborate effectively with multidisciplinary teams
• Familiarity with case management software and documentation practices
